Tooth wear can look cosmetic, but it is often a functional problem

Teeth may shorten, chip, become sensitive, or start to fracture. Treatment is not “one size fits all”. The right approach depends on the cause of the wear, the bite, the amount of remaining enamel, and what you want to achieve.

At Harcourt Dental clinic, worn teeth treatment is planned and led by Dr Kevin Gilmore (prosthodontist). The focus is on stabilising the cause, rebuilding function and comfort, and achieving a natural-looking result that can be maintained long-term.

Why do teeth wear down?

Tooth wear is usually due to one or more of the following:

Grinding or clenching (often at night).
Acid erosion from diet or reflux.
Tooth-to-tooth wear over time.
Bite changes and heavy contacts.
Older restorations are breaking down.

What are the treatment options for worn or damaged teeth?

Most patients benefit from a staged approach. We start conservatively, test changes where appropriate, and only move to more extensive restorations when the foundations are stable.

What does stabilisation and protection involve?

Option 1: Stabilisation and protection

For mild to moderate wear, or as the first step in more advanced cases.

Advice to reduce wear drivers (dietary acids, reflux referral where appropriate, habit advice).
Managing sensitivity.
Repairing small chips with composite.
A protective night guard is recommended if grinding/clenching is suspected.

This stage often prevents further damage and buys time while we plan.

What are additive composite build-ups?

Option 2: Additive composite build-ups

Composite is often the most conservative way to rebuild worn teeth because it usually requires minimal drilling.

This may involve:

Edge bonding to rebuild chipped front teeth.
Posterior composite build-ups to restore bite support.
“Test-drive” changes to the bite before final ceramics.

Composite can be an excellent long-term solution in suitable cases and can also serve as a stepping-stone to ceramic restorations if needed later.

What are ceramic onlays and when are they used?

Option 3: Ceramic onlays (cuspal coverage)

When back teeth are worn, cracked, or heavily restored, onlays/crowns can be a very strong and conservative option.

Onlays:

Protect weakened cusps.
Restore chewing function.
It can often be more conservative than full crowns.
Can be very durable when the bite is well planned.

When are dental crowns recommended?

Option 4: Crowns (when full coverage is needed)

Crowns are used when a tooth needs full coverage for strength, shape, or long-term stability. They may be recommended when:

The tooth is heavily filled or structurally compromised.
There are repeated fractures.
The wear is advanced, and space needs to be created reliably.
Aesthetics and strength must be combined predictably.

A crown should not be the automatic first step for worn teeth. The planning comes first.

What does adjusting the bite involve?

Option 5: Creating space by adjusting the bite (planned bite changes)

One of the biggest challenges in tooth wear is that there may be very little space to rebuild the teeth. In those cases, we may need to create space carefully and predictably. This can involve:

Staged composite build-ups to establish a new bite position.
Temporary “trial” restorations to confirm comfort and function.
A planned sequence so that changes are controlled and reversible early on.

Can orthodontics be part of the treatment plan?

Option 6: Orthodontics as part of the solution (when appropriate)

Sometimes, tooth alignment makes restorative treatment more conservative. Orthodontics may be recommended to:

Reduce destructive contacts.
Improve tooth position before restorations.
Reduce the amount of drilling required.
Improve long-term stability.

What is multidisciplinary care and when is it needed?

Option 7: Multidisciplinary care (when needed)

Worn teeth can overlap with other issues:

Gum disease (periodontal treatment and stabilisation).
Cracked teeth or nerve symptoms (endodontic input).
Missing teeth (implants or bridges).

Where appropriate, treatment is planned as a coordinated sequence, rather than isolated procedures.

How do we decide which treatment is right for you?

Your assessment typically includes:

Detailed clinical examination and bite assessment.
Photographs and digital scans (as required).
Discussion of wear causes and risk factors.
A staged plan with options, timelines and costs.

Where a bite change is planned, we will often “test” the changes before committing to final ceramic restorations.

Longevity and maintenance

Worn teeth treatment is an investment. Long-term success depends on:

Controlling the cause (grinding/erosion).
Appropriate material selection (composite vs ceramic).
Regular hygiene and review.
Protective appliances (often a night guard).

We will advise you clearly on what maintenance is needed for your particular plan.
